All information in written form as well as that discussed between client and therapist are kept confidential. Information will not be released without written consent from the client, unless required by law.

 

            If you have a fever, an allergic reaction, a severe sunburn or a skin rash the day of your appointment, please cancel. Massage therapy can make these conditions more severe. If you suspect you have an infectious condition or have been diagnosed by a doctor with an infectious condition, please cancel any appointments until your condition has been resolved and a doctor has cleared you for massage therapy.

            

            Appointment cancellations must be made a MINIMUM of one full day before a scheduled appointment. If a client does not show up for their appointment or cancels with less than the minimum required notice, they will have to be charged for the missed appointment. Cancellation fees cannot be paid by insurance benefits, and must be paid by the client. The cancellation fee must be paid prior to another treatment. Cancellation fee is charged at the discretion of the therapist. 

 

            Insurance information will be used to E-bill for Therapeutic Massage Therapy only. Insurance will not be billed for missed appointments or cancellation fees. Insurance will not be billed for anyone other than those named on the policy. You have authorized Emma Robar to bill your insurance policy, and receive direct payment from your benefits. You are responsible to pay any outstanding amount not covered by insurance. You are responsible to pay if your E-claim is not accepted.
